Patients now need to submit an online form to request a doctor’s appointment. Please click on the link opposite which says 'contact us online' to request an appointment.  If you are unable to complete the form yourself, please call reception who will complete the form on your behalf.

Once the form has been submitted, the form will be triaged by the doctor and you will be contacted with an appointment suitable to the clinical need of your problem.

You can submit a form Monday to Friday 7am to 7pm (unless we have reached capacity for requests that day). All forms submitted before 2pm will be triaged on the same day. Forms submitted after 2pm may be dealt with on the next working day.

However you choose to contact us, we may offer you a consultation:

• by phone
• face to face at the surgery
• on a video call
• by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

To cancel your appointment:

• use your NHS account (through the NHS website or NHS App)
• using the GP online system – SystmOnline
• phone us on 0113 2310626, Monday to Friday from 8am
• reply CANCEL to your appointment reminder text message

If you need medical help now, use NHS 111 online or Call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Please tell us:

• if there’s a specific doctor, nurse or other health professional you would prefer to respond
• if you would prefer to consult with the doctor or nurse by phone, face-to-face, or by text or email
• if you need an interpreter
• if you have any other access or communication needs

The practice provides home visits for patients who are clinically unable to leave their home and for whom assessment at the practice is not appropriate.

Home visits are an important part of general practice but are a limited clinical resource. They should be used where there is a genuine clinical need to assess a patient in their home.

A home visit is not normally provided because it is more convenient for the patient, because transport is difficult, or simply because the patient is acutely unwell.

If you are clincially unable to leave your home and need to see a healthcare professional, we will phone you first to understand what you need. To request a home visit, its helpful to call the surgery as soon after 8am as possible.
